In Care Coordination we strive to meet patients where they are. Every day we guide patients through socioeconomic disparities, as well as medical barriers. We encourage patients to maintain self-care by staying up-to-date on physicals, bloodwork, vaccinations, and preventative diagnostic tests. One of our goals is to identify and reach out to patients to give them the tools they need to be the healthiest they can be. An example of this would be through our diabetes education program. We love helping patients solve problems and find ways to reduce or even prevent those problems in the future. Our specialty is patient education, because we truly believe an educated patient is an empowered patient.
We want to help you be successful in managing your blood sugars. We offer monthly diabetes group classes at the Elizabethton Medical Care location. At these sessions, you will learn the importance of meal planning, carb counting, healthy eating, and activity for managing your blood sugar.
