Amerigroup No Longer Accepted as of Friday, July 1st, 2016

DO YOU WANT TO REMAIN A MEDICAL CARE PATIENT?

Beginning Friday, July 1st, 2016, Medical Care will no longer accept AMERIGROUP insurance.

IF YOU HAVE AMERIGROUP:Between July 1st and July 31st, you must call 1-800-878-3192 and change to BlueCare or United Healthcare with Medical Care, PLLC as your Primary Care Provider (PCP).

If you are unsure of which TennCare or PCP you have, please call TennCare Solutions at 1-800-878-3192 to find out. For more information, please click here.

New Paperwork for 2016

Save time at your next visit by printing out and completing your Patient Registration Form and Patient Privacy Form in advance. Each of these forms must be filled out once per calendar year.

You will be asked to fill out this paperwork upon your first visit to any Medical Care office in 2016. We have also posted the exact same check-in forms on our website for you to print and fill out in advance. This will help you save time at your next visit with us.

If you want a fighting chance against the flu this season, Medical Care recommends that you get an annual flu shot. Because it takes about 2 weeks for the flu vaccine to take effect, it’s best to get your flu shot early in the season. Flu shots are available by walk-in or appointment. New TennCare Requirement

Is a Medical Care provider name on your insurance card?

Effective AUGUST 1st, 2015,  TennCare will not allow us to see or treat any patient if one of our providers is not on your insurance card.

If you have BlueCare or United Healthcare:

Visit us at any Medical Care location. Ask the receptionist for a completed form. Sign & return this form, and Medical Care can FAX to switch providers (PCP) on your behalf. You may also call your Insurance provider directly:

  • BlueCare: Please call 1-800-468-9698 to change PCP.
  • United Healthcare: Please call 1-800-690-1606 to change PCP.

If you have Amerigroup:

You must call 1-800-878-3192 and change to BlueCare or United Healthcare to be able to have Medical Care as your Prmary Care Provider.

To continue receiving medical attention at Medical Care, this information must be changed before AUGUST 1st, 2015.

Registration Forms

Save time at your next visit by printing out and completing your Patient Registration Form and Patient Insurance Form in advance. Each of these forms must be filled out once per calendar year.

Medical Records Release Form

If you are a new patient or have been seen by another physician, we will need your medical records to provide you with the best health care. Avoid a trip into our office by printing out this Medical Records Release Form and mail, fax, or deliver it to your previous physicians so we will have your complete medical history ready for your next visit. You may also use this form to request that records be sent from Medical Care to another physician’s office.

Viewing forms may require Adobe Acrobat Reader. The program is available via free download at Adobe.com.
